As part of your testing and treatment program, co-payments will be required by imposing the following flat fee schedule. You will be responsible for paying these co-payment fees to your treatment provider at the time in which the service is provided:

| Mental health group session | $5 per session |
| Mental health individual session | $10 per session |
| Mental health family session | $10 per session |
| Mental health assessment report | $10 per report |
Please note that your requirement to attend substance abuse and/or mental health treatment is not satisfied if you do not have the money in hand to make the co-payment at the time of service. The requirement must be satisfied, which includes your financial responsibility.
